A JPEG format of your company logo is needed for the embroidery digitization. The logo needs to be saved in a folder on your computer prior to completing this form.

Click on Browse..., select the JPEG file of the logo you want to appear on the apparel and click "Open". This will attach the image to the set up form.

Midwest Online Embroidery will charge a one time fee of $100 for the embroidery digitization setup. There are no recurring charges for your Online Company Store. The single charge will appear on the credit card statement as Midwest Online Embroidery.

Help
The Card Identification Number is the group of numbers immediately following your credit card number. These numbers are used by your credit card company to help prevent fraud. Because the Card ID number is not printed on your receipts, it helps ensure that someone is not using your credit card information fraudulently.

American Express- 4 digits on front of card.
Discover, Mastercard, and VISA- last 3 digits on back of card.

Note: The digits are printed on your card, rather then stamped in the plastic like the digits in your credit card number.
